Care Navigators / Receptionists

The Receptionists at Riverside Surgery are better known as Care Navigators who are here to help you in complete confidence, whether over the phone or at the reception desk. All members of staff are bound by rules of confidentiality like the Doctors and Nurses. Care Navigators can occupy many roles and play a crucial part in helping people get the right support, at the right time, to help manage a wide range of needs. Our Care Navigators have been given specialist training to help them direct patients to the right health professional first time. This training allows our team of front line staff to provide you with more information about local health and wellbeing services, both within and outside of primary care, in a safe, effective way. It means that you will find it easier to get an appointment with the correct professional when you need one.

Care Co-ordinators

Care Co-ordinators at Riverside Surgery help to co-ordinate care across the health and care system, helping people make the right connections, with the right teams at the right time. Our Care co-ordinators are effective in bringing together multidisciplinary teams to support people’s complex health and care needs. They can be an effective intervention in supporting people to stay well particularly those with long term conditions, multiple long-term conditions, and people living with or at risk of frailty.

Document Management Team

The Document Management Team are responsible for Scanning & Processing all incoming Hospital Correspondence that the practice receives in the post, digitally or dropped-off by patients.

Clinical Coding Team

The clinical coding team are  responsible for reviewing and accurately entering medical information to patient records. They ensure that diagnoses, treatments, test results, and referrals are coded. This maintains accurate patient records, supports clinical care, reporting, audits, and ensures the surgery meets NHS data and quality requirements.

Business Support Team

The Business Support Officers assist the Business Manager and the Doctors, overlook processing and management of the referral letters, insurance reports, and medical reports.

Prescriptions Management Team

The Prescription Management Team are responsible for Processing all incoming Repeat Prescription Requests that the practice receives in the post, digitally or dropped-off by patients.
