Social Prescriber
At Riverside Surgery our Social Presciber is a key component of personalised care. Social prescribing and community-based support is part of the NHS Long Term Plan commitment to make personalised care business as usual across the health and care system. Personalised care means people have choice and control over the way their care is planned and delivered, based on ‘what matters’ to them and their individual strengths and needs. We believe this represents a new relationship between people, professionals and the health and care system. It provides a positive shift in power and decision-making that enables people to feel informed, have a voice, be heard and be connected to each other and their communities.
